Refund/Exchange Policy  
Refunds and/or exchanges will only be issued if the product is returned to us in the same condition in which you received it.  No refunds or
exchanges will be offered where a product becomes outdated due to a guideline change.

Order Cancellation Policy
Please submit any cancellation via email to nysdf@nysdental.org, or call on our business phone at 800-255-2100 ext.222.

Email cancellations
must contain the following: first name, last name, order number and email address. This information must be identical to the
information originally submitted with your order. Please provide contact information so that we may contact you if we have questions
concerning the cancellation of your order.

Every effort will be made to accommodate the cancellation of your order, providing your order has not already been charged and/or shipped out.
In the event  your order is shipped after a cancellation request is received (via email or phone), we will gladly refund the complete amount
charged, once you have returned the order in full. If you have already completed the online exam associated with the course for which you have
registered, a refund request will not be honored.

Shipping costs
Customer assumes all shipping costs in returning a product to us, and we assume the cost of shipping the product back to the customer. All
products are shipped USPS unless a rush is requested. The cost of a shipping upgrade will be charged to the customer prior to shipment.
Privacy Policy
This Privacy Policy explains our policy regarding the collection, use and disclosure of your personal information. As we update and expand our
services, this policy may change, so please refer back to it periodically. By accessing our Web site or using our services, you consent to our
information practices.

Personal information means information that can be used to identify and contact you, specifically your name, postal delivery address, e-mail
address, payment method (e.g., credit card or debit card) and telephone number, as well as other information when such information is
combined with your personal information.

When you register or re-register at our Web site, we ask you to supply us with personal information so that we can provide, enhance and
personalize our services and marketing efforts. For similar reasons, we may supplement the personal information you provide with publicly
available information about you. We use this personal information to process your registration, your order and payments, and to communicate
with you on these and other topics. We also use your email address to send you newsletters, as well as messages about New York State Dental
Foundation special offers, promotional announcements and consumer surveys.  Occasionally, we may send these materials to you via postal
mail, using the shipping address provided.

The New York State Dental Foundation Web site contains links to sites operated by third parties whose policies regarding the handling of
personal information may differ from ours. While we endeavor to associate with reputable sites, New York State Dental Foundation cannot be
responsible for the information handling practices of these other sites.

As you browse the New York State Dental Foundation site, cookies are placed on your computer’s hard drive (cookies are small pieces of data
that are sent to your browser from a Web server and stored on your hard drive for record keeping purposes). In general, cookies can make the
web more useful by storing information about your preferences on a particular site.  Cookies in and of themselves do not personally identify
users, although they do identify a user’s computer. If your browser is set to not accept cookies, you may not be able to use the New York State
Dental Foundation site.
